Lynx Alumni Program Conceptual Framework

Our program will offer the following components to Clients and Families:

  • Registration in program, graduation from active intensive cm
  • Access to case management and/psychiatrist in an emergency or crisis or as needs change or arrive
  • Referral and support to family doctors, nurse practitioners and family health teams
  • Network of telephone and email support from the program and peers
  • Group Meetings every 6 weeks (to start)
  • Informal psycho-education on an as needed basis to the group or offered on an individual basis
  • Initially, the program will provide social recreation opportunities as the group deems
  • Self-driven alumni related activities
  • Opportunity to advocate on issues relevant to their wellness
  • Contracts with clients for graduation into the alumni program as well as ongoing review of contract.
  • Rapid re-entry into alumni program with original case manager and psychiatrist where possible, on a short-term basis to be determined by the treatment team.
  • Potential involvement and development through mentoring program and peer support
